چکیده
Abstract Aim Encapsulating peritoneal sclerosis (EPS) is a rare morbidity associated primarily with dialysis. Definitive diagnosis and management can often be prolonged complicated. The pathogenesis two-hit hypothesis of inflammation myofibroblast differentiation. This report aims to demonstrate the EPS provide insight into management. Method We present unique case patient who follows ‘classical’ disease course before developing concomitant pericardial sclerosis, presenting cardiac tamponade. explore proposed pathophysiology for both uncommon pathologies in same patient. Results A 45-year-old male was treated went on develop sclerosis. Following referral our centre, underwent semi-elective surgical enterolysis peritonectomy EPS, excision all sclerotic obstructing membrane. Two weeks following surgery, he developed tamponade cardiovascular compromise, unresolved by two separate episodes pericardiocentesis, leading intervention. pleural window created via open thoracotomy 800ml thick clotted blood removed from cavity. Histology demonstrated Conclusions given Uraemia may common mediator peritoneum pericardium, predisposing membranes identical embryological origin. Pericardial patients but not manifest itself clinically large. It also demonstrates that effective intervention lead excellent outcomes.

INTRODUCTION Encapsulating peritoneal sclerosis (EPS) is a rare but devastating complication of peritoneal dialysis (PD). Tamoxifen has been generally well-tolerated, even without randomized controlled trials. CASE PRESENTATION Herein, we report a case of a patient undergoing 12 years of PD who developed EPS and calciphylaxis simultaneously. We also provide a comprehensive discussion about th...

Encapsulating peritoneal sclerosis (EPS), is a rare but devastating complication of long-term peritoneal dialysis (PD) with a high mortality rate. The incidence is between 0.5 and 3.3%, decreasing with time. EPS is defi ned as a clinical syndrome with major signs of gastrointestinal obstruction, infl ammatory parameters, radiological and macroscopic changes. Duration of treatment and cessation ...

cified peritoneum. Diagnosis of EPS was made. Steroids (0.5 mg/kg/day) and Tamoxifen 20 mg twice per day were started. The patient is now on a normal diet and CRP has returned to normal. A definitive diagnosis of EPS is done at laparotomy or laparoscopy but may be strongly supported by a combination of clinical and radiologic features [1] . Computed tomography has largely been used to delineate...
متن کاملEncapsulating peritoneal sclerosis following a peritoneal foreign body reaction to Dacron fibres—a case report
The aetiological factors and the pathophysiological mechanisms of encapsulating peritoneal sclerosis (EPS) have not been fully elucidated. We present a patient on continuous ambulatory peritoneal dialysis whose peritoneal catheter was exchanged due to repeated episodes of bacterial peritonitis. Immediately afterwards, he experienced severe abdominal pain, nausea and fever. Peritoneal biopsy, ta...
